Assessments

Together! Healthy Knox uses the four assessments of the Mobilizing for Action through Planning and Partnerships (MAPP) model for community health planning (see figure). MAPP was developed through a cooperative agreement between the National Association of County and City Health Officials and the Centers for Disease Control and Prevention.

The purpose of the assessment is to provide a snapshot of the factors affecting the health status of Knox County residents, to provide useful information for local programmatic and fiscal decision-making and to inform the development of a strategic community health improvement plan. Each assessment will yield important information for improving community health, but the value of the four MAPP Assessments is multiplied by considering the findings as a whole. Disregarding any of the assessments will yield an incomplete understanding of the factors that affect the local public health system and, ultimately, the health of
the community. As of fall 2010, the Community Themes & Strengths Assessment and the Community Health Status Assessment have been completed.  The Forces of Change Assessment and the Local Public Health System Assessment are scheduled to be completed by spring 2011.
